Past graduation/ statement of results

Copies of past graduation certificates/statement of results can be obtained by completing the ‘Application for a copy of results document/s’ which is available on our Applications and forms page.

The following must be included with your application:

1. Proof of identity e.g. copy of driver’s license, birth certificate, passport etc.
2. Change of name documentation( if applicable) e.g. marriage certificate, deed poll certificate
3. Payment of $29.00 (incl. GST) per certificate
